PCL-5 PTSD Test — Trauma Symptom Screening

The PCL-5 (PTSD Checklist for DSM-5) is a 20-item screening questionnaire assessing trauma-related distress over the past month. It can indicate whether trauma symptoms may be worth discussing with a professional — it does not diagnose PTSD.

What the PCL-5 measures

The PCL-5 covers intrusion, avoidance, negative alterations in cognition and mood, and hyperarousal — the symptom clusters associated with post-traumatic stress in DSM-5. Each item uses a 0–4 distress scale for the past 30 days.

Who should take this screening

Consider the PCL-5 if you have experienced a traumatic event and notice persistent re-experiencing, avoidance, mood changes, or heightened alertness that affects daily life. Trauma content can be distressing — proceed at your own pace.

How PCL-5 scoring works

Scores reflect self-reported symptom burden. Clinicians often use provisional cut-offs around the low 30s as a prompt for further assessment — not as a definitive diagnosis.

Lower scoresFewer endorsed symptoms — still discuss concerns if they affect functioning
Mid-range scoresMay indicate it is worth discussing trauma symptoms with a professional
Higher scores (e.g. ~33+)Often used in practice as a prompt for formal trauma assessment — not a diagnosis on its own
